How to apply:

Please complete steps 1 and 2 and we will contact you to  schedule an intake appointment either online or on campus.

Steps to follow:

Step 1 □ 

Complete the Application for Services and Intake Screening form

Step 2 □ 

Submit copies of your Verification of Disability

(Email completed copies to MCACCESS@VCCCD.EDU)

  • Disability verification is generally a letter or document provided by a qualified professional that identifies the nature of the condition and the resulting functional limitations. IEPs and 504 plans are also considered disability verification forms.

  • Students with an IEP or 504 plans from high school: Please submit a complete (include all pages) IEP or 504 plan, and a copy of a recent psycho-educational assessment report, if available.

  • If you don’t already have a verification of disability, we have a Disability of Verification Form available to assist you with obtaining disability documentation from a healthcare professional.

Step 3 □ 

Once steps 1 and 2 have been completed, we will call you to set up an intake appointment

   

Step 4 □ 

If you are a current high school student or recent graduate, submit an unofficial transcript as well.
